Appointment of Agent & Release of Information Privacy Act 1988
To: Collection & Recovery Options Pty Ltd A.C.N. 163 752 029
- I/WE the person or incorporated body described above hereby appoint Collection & Recovery Options Pty Ltd A.C.N. 163 752 029 (“CRO”) and its Directors and Senior Managers jointly and severally to act on my/our behalf in all or any of the following matters:
- To apply and receive a copy of my/our Credit Report.
- To view the credit information file held by any Credit Reporting Agency or Credit Provider.
- To seek to procure any alteration on the credit information file held by any Credit Reporting Agency or Credit Provider.
- To Negotiate any reasonable settlement of any debt due with any person who has provided credit to me/us or any third party for whom I/we have provided a guarantee or indemnity.
- To vary the terms on which any credit provided to me/us or any third party for whom I/we have provided a guarantee or indemnity so as to enable me/us to satisfy any default or to otherwise compromise our obligations.
- Seek legal advice on my/our behalf.
- To seek and receive information of any description or type including but not limited to Taxation Information, Banking Information , and all other documentation and files of any nature or kind relating to or concerning me/us) held by a Third Party.
- I/WE hereby authorise CRO and its officers, agents, employees and consultants to hold full and frank discussions pursuant to this appointment with any Credit Reporting Agency or Credit Provider, Legal Advisers/Consultants or Government Agency including the Australian Taxation Office and any State office of Revenue or other Third Party (howsoever described or constituted) with whom I/we have had dealings or entered into transactions of any kind with regarding my/our affairs, dealings, transactions and financial position and any other matter CRO in their sole and unfettered discretion seek to enquire about concerning me/us and further authorise and direct you to disclosed in the course of those discussions any and all information and to provide documents or copies thereof concerning the said affairs, dealings, transactions and financial position and any other matter CRO in their sole and unfettered discretion seek to enquire about concerning me/us to CRO. I/We further confirm that by execution hereof that I/we have authorised CRO to act as my agent to exercise my/our rights to obtain access to all of my/our information and documents in your possession or control.
